At Gloria & Sullivan-Cook, LLC, we offer managed care specifically designed for individual lifestyles. Whether due to a prolonged illness, incapacity, disability, or through the natural challenges of aging, daily responsibilities can become difficult. We coordinate medical, personal and social service resources to enhance the quality of life for our clients.

GSC provides services including, but not limited to: Guardian, Conservator, Power of Attorney, Trustee, and Personal Representative as well as Fee for Service for Geriatric Care Management.

GSC specializes in assisting the elderly and incapacitated in maintaining their independence and can help ease the transition to a new setting, if necessary. A Care Manager creates and coordinates a customized plan of care and services to meet the unique needs of each individual and family. The Care Manager monitors the plan to ensure that loved ones receive continuous care, whether at home, in a residential care home, or a skilled nursing environment. We can also assist with ALTCS applications when needed.

Our Care Managers are professionally trained and specialize in working with the memory impaired. We take the time to get to know our clients and understand their concerns, needs and wants. We aim to provide the same care as a family member would when taking care of their loved one.
